Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) and/or Licensed Vocational Nurse (LVN) for our Hospice division. The ideal candidate will provide high-quality nursing care to patients in their homes or facilities, ensuring their comfort and well-being. This role requires a strong understanding of various nursing systems and practices, particularly in home care settings. The discipline will work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans.
Experience
- Current Registered Nurse RN and LVN license in good standing.
- Experience in Hospice and home health or community health nursing preferred.
- Strong communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work independently while managing multiple patient cases effectively.
Job Description for RN (per diem):
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Develop, implement, and evaluate individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed and acute pain management.
- Provide specialized care for patients requiring memory care, ostomy care, and critical care interventions.
- Educate patients and their families on health management, medication administration, and self-care techniques.
- Collaborate with healthcare teams to coordinate patient care and ensure continuity of services.
- Maintain accurate documentation of patient progress, interventions, and outcomes in compliance with healthcare regulations.
- Utilize coding systems for accurate billing and reporting of services provided.
Job Description for LVN (per diem):
- Provides direct patient care including specialized duties related to end-of-life care.
- Implements current nursing practice following a comprehensive assessment and the plan of care.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Provides appropriate pain/symptom management. Evaluates patient response to treatments/medications.
- Performs other duties as assigned by the registered nurse.
Job Type: Part-time
Pay: $35.00 - $150.00 per hour
