Responsibilities
- Provide direct nursing care in accordance with physician orders and individualized care plans.
- Administer medications, treatments, and prescribed nursing interventions.
- Monitor and document patient conditions, vital signs, and responses to treatment.
- Perform nursing procedures including wound care, catheter care, tracheostomy care, G-tube care, and medication administration.
- Assist clients with activities of daily living while promoting independence, dignity, and quality of life.
- Collaborate with physicians, registered nurses, caregivers, case managers, and family members to coordinate care.
- Maintain accurate and timely documentation in compliance with agency policies, Medicaid requirements, HIPAA regulations, and state standards.
- Participate in assessments, care planning, and ongoing evaluations.
- Educate and support clients, families, and caregivers.
- Respond appropriately to changes in condition and emergency situations.
Preferred Experience
Experience in one or more of the following areas is preferred:
- Home Health Care
- Private Duty Nursing (PDN)
- Pediatric Nursing
- Long-Term Care
- Assisted Living
- Developmental Disabilities (ID/DD)
- Memory Care and Dementia Care
- Hospice Care
- Medication Administration and Management
- Wound Care
- Electronic Medical Records (EMR/EHR)
Qualifications
- Active Virginia Licensed Practical Nurse (LPN) License in Good Standing
- Current CPR/BLS Certification
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Communication Skills
- Ability to Work Independently and as Part of a Multidisciplinary Team
