Job Summary
We are seeking energetic and detail-oriented Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide hospice continuous care services. This role involves delivering specialized nursing interventions in patients’ homes or care facilities, ensuring comfort and dignity for individuals receiving end-of-life care. Candidates must hold an active nursing license and be committed to making a meaningful difference in patients’ lives.
Duties
- Provide comprehensive hospice continuous care, including symptom management and comfort measures tailored to each patient’s needs.
- Conduct thorough patient assessments, vital signs monitoring, and documentation of clinical findings .
- Administer medications, following strict protocols and safety standards.
- Manage complex cases involving acute pain, dementia, Alzheimer's disease, and other chronic conditions with compassion and professionalism.
- Collaborate with interdisciplinary teams.
- Educate patients and family caregivers on disease progression, medication administration, and symptom control strategies.
- Ensure compliance with HIPAA confidentiality policies, infection control procedures, and documentation accuracy.
Experience
- Valid nursing license (LPN or RN) for at least 1 full year required.
- Background in long-term care settings including skilled nursing facilities, assisted living, or home health is advantageous.
- Strong patient assessment skills with the ability to handle urgent situations such as airway management or acute pain episodes confidently.

Pay: $30.00 - $32.00 per hour
