Licensed Physical Therapist Assistant
We are seeking to add Physical Therapist Assistants with a current license in the state of Illinois to provide ongoing therapy services in the home for children from birth to 3 years of age who are enrolled in the Illinois Early Intervention Program.
- PTAs will be required to have/maintain/obtain Illinois Early Intervention Physical Therapy Assistant Credentials (Advance Rehab Services will assist non-credentialed PTAs in obtaining their Illinois Early Intervention Physical Therapy Assistant Credential).
- PTAs will be required to show proof of and maintain up to date Malpractice/Liability Insurance
- PTAs will be expected to work as a team member to maximize efficiency and quality of patient treatments while following and meeting all agency and Provider Connection Early Intervention requirements.
- PTAs will provide ongoing therapy services as instructed by our Physical Therapists, follow the Individualized Family Service Plan, track, monitor and record all services provided, as well as progress, client response, and/or changes in condition and must meet with their supervisor on a monthly basis creating/maintaining a curriculum based on meeting each child’s goals.
- PTAs will be expected to schedule and arrive to work consistently on time, prepare and submit documentation completely and on time, and maintain client confidentiality.
