Responsibilities
- Assess patients’ physical capabilities and develop personalized care plans that promote comfort and functional independence
- Implement physiotherapy techniques including trigger point therapy, orthopedics, and geriatrics-focused interventions to alleviate pain and improve mobility
- Coordinate discharge planning with interdisciplinary teams to ensure seamless transition of care from hospital or inpatient settings to home or community environments
- Provide care across various settings such as home health visits, outpatient clinics, hospitals, and inpatient units
- Educate patients and caregivers on physiotherapy exercises, mobility aids, and safety measures to enhance ongoing care at home or in community settings
- Document patient progress accurately using electronic medical record (EMR) systems and communicate updates effectively with healthcare teams
- Stay current with medical terminology, anatomy knowledge, and physiotherapy advancements to deliver evidence-based treatments
Experience
- Proven experience in acute care hospital environments with a strong understanding of inpatient and outpatient physiotherapy practices
- Hands-on experience with discharge planning, home & community care, and outpatient therapy services
- Familiarity with EMR systems for documentation and patient record management
- Background working with diverse populations including pediatrics, geriatrics, orthopedics, and patients requiring trigger point therapy
- Knowledge of physiology, anatomy, medical terminology, and specialized therapies relevant to hospice care
- Demonstrated ability to collaborate effectively within multidisciplinary teams to optimize patient outcomes
- Prior experience in home health or clinic settings is highly desirable

Pay: $44.99 - $54.18 per hour
