Summary:
The Targeted Case Manager shall provide case management services as directed by the Targeted Case Management Supervisor/Director of Case Management, which shall include assisting clients in gaining access to needed financial and insurance benefits, employment, medical, social, education, assessment of functional abilities and needs, and other services. These support services include working with the client and his/her natural support system to develop and implement the service plan. It also includes follow-up to determine the status of services and the effectiveness of activities related to the successful implementation of the service plan toward enhancing the recipient’s inclusion in the community.
ESSENTIAL FUNCTIONS:
Developing, Implementing and Reviewing the Service Plan, as well as working with the client and family to address issues related to the services. Services where the family is involved must be clearly directed to meeting the identified needs of the client.
Linking and facilitating with appropriate services and resources identified in the service plan.
Advocating for the acquisition of services and resources necessary to implement the service plan.
Coordinating the delivery of services as specified in the service plan, with the help of the client, his/her family and his/her natural support system.
Monitoring service delivery to evaluate client’s progress.
Arranging for and coordinating after care services upon discharge from a residential or inpatient facility when discharge planning is not covered by the facility’s per diem.
Demonstrates knowledge of the Medicaid manual and all updated revisions.
Completes ongoing chart audits to ensure compliance and quality assurance of services provided.
Demonstrates knowledge of DSM-IV and DSM-V.
Demonstrates proficiency in utilizing community resources as supportive services.
Serving as a liaison for the client, family, referral source and other social service agencies.
Ability to report to work as scheduled, in a timely fashion and will an attitude have oriented toward service.
Presents in a professional manner in the community, in person, on telephone and in written communication including times of extreme stress and when others are upset.

EDUCATION AND/OR EXPERIENCE
Graduation from an accredited college or university with a minimum of a Bachelor’s Degree in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or related field with at least 1-year experience working with the specific population they are applying to work with (i.e. adults with serious mental illness)
- Must complete an AHCA approved mental health targeted case management training within 3 months of initially supervising case managers.
- Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population.
- Must have knowledge of the principles and practices of the discipline.
LANGUAGE SKILLS: Fluent in English and Spanish (or other languages) helpful. Must be able to read and comprehend simple instructions, short correspondence, and memorandums. Agility to effectively communicate information both orally and written on one-to-one and small group situations to clients and other employees.
MATHEMATICAL SKILLS: Ability to add, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio and percent.
REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
ORGANIZATIONAL SKILLS: Must be organized and able to schedule workload appropriately without hindrance to client appointments etc.
DECISION MAKING: Must be able to work independently with competent decision making using established criteria and guidelines.
OTHER SKILLS AND ABILITIES: Computer Skills: To perform this job successfully, an individual should have knowledge of Accounting software; Database software; Internet software and Word Processing software.
