Provide direct resident care services while following individualized plans of care

Administer medications in accordance with all standards of practice, state regulations, and community policies, including proper documentation and follow-up

Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living and/or Director of Memory Care

Observe residents for changes in condition and report concerns promptly to leadership

Document all relevant observations, communications, clinical procedures, and daily care by the end of each shift

Maintain and update individualized care plans for each resident

Provide compassionate support and excellent customer service to residents and families

Assist with coordination of resident care and communication among departments

Support a positive, team-focused environment and assist with mentoring care staff as needed

Perform other duties as assigned by supervisor
This position works under the direction of the Director of Assisted Living and/or Director of Memory Care while exercising independent judgment and professional decision-making. The LPN may assist with supervision, direction, training, and support of other team members as needed.

High school diploma or equivalent (GED) required

Current Licensed Practical Nurse (LPN) license in the state of Florida required

Current CPR certification required

Minimum of one year of related nursing experience preferred

Strong clinical, communication, and organizational skills

A passion for serving seniors and providing exceptional resident care

Ability to sit, stand, walk, bend, stoop, kneel, crouch, climb stairs, and reach throughout the workday

Ability to lift up to 25 pounds unassisted

Ability to push, pull, reposition, and transfer residents with assistance or appropriate equipment
