What You’ll Do
The REAYL Access Housing Options YHDP Rapid Rehousing Program is seeking a Case Manager to provide Critical Time Intervention supportive services to program participants. Using a housing-first approach, the Case Manager will provide supportive services to single young heads of household (ages 18 to 24) who enter the emergency shelter system in Philadelphia. In collaboration with the Housing Stabilization Specialist and Employment Specialist, the Case Manager will work to swiftly connect clients to permanent housing with no eligibility requirements related to sobriety, income, employment, or criminal history and to promote independent living skills and increase housing stability. The Case Manager will conduct monthly home visits and enter all relevant information in the AWARDS database.
Your responsibilities will include:
- Conducting assessments, formulating individualized service plans, and providing referrals and ongoing support to assist tenants in developing skills and achieving personal goals associated with housing stability and well-being.
- Serving as the foundation of the supportive services made available to clients, such as connecting them to community-based services and resources that are aligned with their IHSP.
- Facilitating a range of support groups and workshops designed to help tenants live as independently as possible.
- Serving as a liaison with medical, mental health, and substance abuse providers to ensure clients receive a high level of care.
- Providing assistance with educational and employment linkages.
- Working with property management to support tenants in all aspects of maintaining permanent housing.
You’re a great fit for this role if you have:
- Bachelor's Degree in Social Work or a related field required, MSW preferred.
- Minimum two years of experience in case management, assessment, counseling, and crisis intervention.
- Experience working with the homeless population is a plus.
- Knowledge of AWARDS is a plus.
- Computer literate, specifically with Microsoft applications, required.
Compensation
Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws.
