- Pay Range: $1,981.20 - $2,080.26
- Schedule: 8AM-5PM
- Contract Length: 13 weeks
- Candidate Type: Local and traveler friendly
- Requirements: 1 year of Medicare certified Home Health experience. 1 year of HCHB EMR and OASIS experience.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team
- Provide skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy
- Perform OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirements
- Educate patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence
- Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes
- Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care
