Licensed Practical Nurse (LPN/LVN)

Position Summary
We are seeking a qualified LPN/LVN to provide high-quality, patient-centered care in a clinical setting. The ideal candidate will have strong assessment, medication administration, and patient communication skills.
Key Responsibilities
- Administer oral, injectable, IV, and topical medications.
- Perform patient assessments and monitor vital signs.
- Conduct medication reviews and identify potential side effects/interactions.
- Perform phlebotomy, vaccine administration, and point-of-care testing.
- Document patient information accurately in EMR systems.
- Educate patients on medications, self-care, and disease management.
- Communicate effectively with patients, families, and healthcare professionals.
- Maintain a clean, organized, and well-stocked clinic.
Qualifications
- Active, unrestricted LPN/LVN license.
- Minimum 1 year of healthcare experience.
- Primary Care, Urgent Care, or Emergency Room experience preferred.
- Experience with phlebotomy, vaccines, and point-of-care testing preferred.
- EMR experience required; Epic experience is a plus.
Note: Candidates whose experience is primarily limited to nursing homes, home health, LTAC, or skilled nursing facilities are not preferred for this role.
