The Care Manager Supervisor will provide administrative oversight to a team of Care Managers whose responsibilities include case coordination for children, adults and/or families in the Care Management Program. Care Managers serve members who are enrolled in Medicaid,have Two or more Chronic Conditions, HIV/AIDS, Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI), Sickle Cell, or Complex Trauma, ages 0-21 (Children’s Program) & 18 to end of life (Adult Program). The Care Manager Supervisor plays a critical role in ensuring the overall goals for coordination,integration and partnership in the delivery of care to children, adults and families with complex needs.
I. SPECIFIC RESPONSIBILITIES
Supervision:
Supervise and support Care Managers through regular individual and team supervision
Oversee caseloads to ensure timely documentation and compliance with regulatory requirements
Review and maintain electronic health records (EHR) for accuracy and completeness
Provide coaching, performance feedback, and professional development support
Assist with audit preparation and ensure program readiness Key Performance Indicators (KPIs,
documentation, reporting)
Participate in field visits, case conferences, and interdisciplinary team meetings (IDT)
Manage staff schedules and coverage; provide direct services as needed
Collaborate with internal teams and community partners to support client outcomes
Training & Development:
Schedule and track required trainings (DOH, Compliance, HIPAA, Health Homes, Relias,
others)
Ensure completion of mandated annual trainings
Facilitate staff training and provide program updates
Monitor and communicate policy or program changes to staff and leadership
Staff Support & Performance Management:
Provide real-time coaching, field-based supervision, and case guidance
Deliver performance evaluations, corrective action, and recognition (with HR support)
Promote leadership development and problem-solving skills among staff

Working Conditions:
This is an office/field-based position, Nassau & Suffolk County as well as the 5 boroughs of NYC.
All office days, staff meetings and supervisory meetings are in-person and or virtual and held at one
of our two locations (Brooklyn/Dix Hills).
New York residency required
III. Qualifications:
Education:
Master’s degree preferred in Social Work, Nursing, Public Health, or related field
Bachelor’s degree required with minimum 4+ years of experience in child welfare, care management,
or a related field
Experience:
2–5 years of supervisory or managerial experience
Experience in Health Home, care coordination, or case management programs
Strong knowledge of DOH regulations, Health Home standards, and compliance requirements.
All Health Homes Serving Children (HHSC) Care Managers Supervisors must:
Complete Children Adolescent Needs Assessment (CANS)NY training
Be CANS-NY certified annually
Meet exam score requirements:
o Supervisors: minimum 80%
o Care Managers: minimum 70%
Skills & Competencies:
Strong leadership, staff development, and performance management skills
Excellent communication, organization, and problem-solving abilities
Experience with electronic health records (e.g., Foothold) and documentation oversight
Ability to manage multiple priorities, deadlines, and audits
Strong crisis intervention and decision-making skills
