MSW (Social Worker)
Location- New York, NY
Salary- $65,000-$69,000
Key Responsibilities:
- Conduct comprehensive psychosocial assessments to identify client needs, strengths, and housing barriers.
- Develop individualized service and housing plans in collaboration with clients.
- Assist clients in securing transitional and permanent housing, including completing applications and coordinating with housing providers.
- Provide ongoing case management, including regular meetings to monitor progress and adjust goals.
- Connect clients to community resources such as mental health services, healthcare, employment programs, and financial assistance.
- Advocate on behalf of clients with landlords, service providers, and government agencies.
- Facilitate life skills training, including budgeting, tenant rights, and independent living skills.
- Maintain accurate and timely documentation in compliance with agency and funding requirements.
- Participate in multidisciplinary team meetings and coordinate care with internal and external partners.
- Support crisis intervention and stabilization as needed.
Qualifications:
- Master’s Degree in Social Work (MSW) from an accredited program.
Pay: $65,000.00 - $69,000.00 per year
