Shift Requirements : M-F 8am-5pm
OnCall Required: Rotating weekends
Productivity: 30 points weekly
License required: RN Licenses in IN and will accept compact license
Mileage Reimbursement: 0.43 Cent per mile
County's Covered:
1 RN-Clark & Floyd counties with addt counties as needed for call in and PTO.
1 RN-Harrison & Crawford counties with addt counties as needed for call in and PTO.
1 RN Washington & Orange counties with addt counties as needed for call in and PTO.
Required Home Health Skills: Wound Care, IV, Drain care, G tubes, basic education. HCHB & Oasis required.
Duties
- Conduct comprehensive assessments of high-risk members and develop individualized, cost-effective care plans in collaboration with primary care providers.
- Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and other healthcare professionals to ensure continuity of care.
- Arrange non-medical support services, including housing and transportation, to improve treatment adherence and member engagement.
- Monitor clinical, functional, and financial outcomes, identify opportunities for health promotion, and intervene promptly to prevent adverse events.
- Advocate for patients by protecting privacy and confidentiality while utilizing strong clinical judgment to analyze complex medical information and coordinate appropriate care.
