POSITION SUMMARY
We are seeking a skilled and compassionate Hospice Nurse (Registered Nurse) to join our team covering the Peninsula. This position includes a blend of patient home visits throughout San Mateo County, quality assurance responsibilities, and in-office support at our Pleasant Hill, CA office. When not conducting patient visits, the nurse will contribute to QAPI activities and office operations to help maintain the highest standards of care across our organization.
RESPONSIBILITIES
Clinical / Field:
- Perform skilled hospice nursing visits and comprehensive assessments in patients' homes throughout the Peninsula
- Provide symptom management, comfort-focused care, and pain control as ordered by the physician
- Administer medications, wound care, and treatments in accordance with the patient's plan of care
- Monitor and document changes in patient condition, symptoms, and disease progression
- Educate patients and families on the disease process, comfort measures, medications, and what to expect
- Provide emotional support and guidance to patients and families navigating end-of-life care
- Collaborate with the interdisciplinary team — physicians, social workers, chaplains, and aides — to ensure coordinated, holistic care
- Maintain timely and accurate documentation in the EMR system
- Report changes in patient condition to the supervising RN Case Manager or Director of Patient Care Services
- Participate in on-call rotation for after-hours patient and family support, as needed
Quality Assurance:
- Review clinical documentation for accuracy, completeness, and compliance with hospice regulatory standards
- Assist with chart audits and corrections as needed
- Identify care gaps and work with the clinical team on QAPI (Quality Assessment and Performance Improvement) initiatives
- Support survey readiness and ongoing compliance efforts, including Joint Commission and state requirements
Office / Administrative (Pleasant Hill, CA):
- Participate in team meetings, IDG (Interdisciplinary Group) meetings, trainings, and quality improvement initiatives
QUALIFICATIONS
- Active California RN license (required)
- At least 1 year of clinical experience; hospice or palliative care experience strongly preferred
- Reliable vehicle and valid driver's license required — must be able to travel throughout the Peninsula
- Strong communication, organization, and critical-thinking skills
- Comfortable managing a full-time caseload independently within San Mateo County
- Compassionate, calm, and steady presence with patients and families during emotionally difficult times
- Experience with or willingness to learn hospice QAPI/compliance processes
- Familiarity with hospice documentation and Medicare hospice benefit guidelines a plus

- Weekday and weekend visit availability, plus on-call rotation
