Job Summary

Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using proper manual handling techniques and equipment like Hoyer lifts.
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems and EMR platforms.
- Support medication administration under supervision, ensuring adherence to HIPAA regulations and maintaining patient confidentiality during documentation.
- Provide specialized care for patients with dementia or Alzheimer's disease, utilizing memory care techniques and engaging them in appropriate activities to promote mental well-being.
- Assist with medical procedures such as catheter care, wound dressing changes, phlebotomy (blood draws), and aseptic techniques to prevent infection.
- Collaborate with interdisciplinary teams in acute care settings like ICU or Level I Trauma Centers, contributing to patient recovery plans in medical-surgical or emergency medicine environments.
- Facilitate safe patient transfers using appropriate transfer methods and equipment while practicing safe lifting practices and heavy lifting protocols.
- Communicate effectively with patients, families, and healthcare professionals to ensure comprehensive patient service and optimal care outcomes.
- Maintain accurate medical records and documentation within EMR/EHR systems while complying with all regulatory standards including HIPAA.
Experience
- Prior experience working in diverse healthcare settings such as hospitals, long-term care facilities, hospice or palliative care units, outpatient clinics, or home health environments is highly preferred.
- Demonstrated proficiency with EMR/EHR systems like Cerner or eClinicalWorks for medical documentation and patient records management.
- Experience working with individuals with disabilities or developmental delays, including geriatrics or pediatrics populations.
- Knowledge of medical terminology related to acute care, geriatrics, trauma centers, ICU procedures, infectious disease care, and post-acute care is advantageous.
- Certification in Basic Life Support (BLS) or CPR is required; additional certifications such as Phlebotomy or medication administration are a plus.
- Familiarity with patient transfer methods and safe handling practices in various clinical environments enhances your ability to deliver exceptional patient care.
- Strong caregiver communication skills combined with a thorough understanding of physiology and anatomy knowledge support effective teamwork and patient engagement.
