Registered Nurse (RN) Case Manager / LCSW – Travel Contract

Contract Length: 13 Weeks
We are seeking an experienced RN Case Manager or Licensed Clinical Social Worker (LCSW) for a 13-week travel assignment in Glen Burnie, MD. This is a weekday-only position with no weekends or holidays, offering an excellent work-life balance.
Position Details:
- Schedule: Monday–Friday
- Shift: 5x8 Days (8:00 AM – 4:30 PM)
- Contract Length: 13 Weeks
- Start Date: July 27, 2026
- No weekends or holiday shifts
Responsibilities:
- Coordinate patient care and discharge planning.
- Collaborate with physicians, nurses, and interdisciplinary teams.
- Assess patient needs and develop individualized care plans.
- Ensure appropriate utilization of healthcare resources.
- Communicate with patients, families, and insurance providers to facilitate quality care.
- Manage cases for medical, surgical, ICU, and IMC patients.
Requirements:
- Active Maryland RN License
- Current BLS Certification
- Previous Case Management experience preferred
- Strong communication, care coordination, and discharge planning skills
