Overview

As an essential part of our home health services, you will promote health, prevent illness, and support recovery through comprehensive patient assessments and targeted interventions.
Duties
- Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories to determine individualized care plans.
- Administer medications, injections, IV infusions, and other treatments following established protocols and ensuring patient safety.
- Manage complex medical procedures such as catheterization, tube feeding, wound care, and sterile techniques with precision and adherence to infection control standards.
- Coordinate discharge planning and collaborate with physicians, case managers, and families to optimize patient outcomes across various settings including outpatient clinics, assisted living facilities, nursing homes, and private residences.
- Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care activities, medication administration, and medical records management.
- Provide education on disease management such as Alzheimer’s care, dementia care, pain management, and chronic condition monitoring to patients and their caregivers.
- Respond promptly to urgent situations through triage skills and emergency response procedures while maintaining compliance with HIPAA regulations.
Skills
- Proficient with Wellsky/Kinnser.
- Proven experience in critical care environments such as ICU or Level I Trauma Centers with a strong understanding of acute pain management and trauma protocols.
- Experience with home health and Oasis Reports
- Extensive knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
- Proficiency in EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for seamless documentation and communication.
- Skilled in performing phlebotomy, venipuncture, specimen collection/processing, and medication administration with aseptic technique.
- Ability to work effectively with diverse populations including pediatrics (neonatal ventilator), geriatrics (memory care), individuals with disabilities or developmental disabilities, and those requiring hospice or post-acute care services.
- Strong assessment skills for working with patients experiencing infectious diseases or requiring ventilator management; experience in emergency medicine or hospital medicine is highly valued.
- Knowledge of home & community care practices such as DME (durable medical equipment), catheter care, NG tube feeding, and oxygen therapy.
- Excellent communication skills for patient education on health coaching topics like nutrition or family planning; capable of working independently while maintaining team collaboration.
