- Collaborate with healthcare providers to develop individualized medication plans.

- Maintain accurate records in accordance with HIPAA regulations.
Experience
- A Doctor of Pharmacy (Pharm.D.) degree from an accredited institution is required.
- Excellent communication skills with a focus on patient service and collaboration within healthcare systems.
