Responsibilities:
Oversee the planning, implementation, and evaluation of rehabilitation services, ensuring compliance with regulatory requirements and organizational standards
Lead a team of rehabilitation professionals, including occupational, physical, and speech therapists, to deliver individualized treatment plans that promote optimal patient outcomes
Develop and maintain relationships with referral sources, including hospitals, physicians, and other healthcare professionals, to ensure a continuum of care for our residents
Analyze and report on rehabilitation data, identifying trends and areas for improvement to optimize program effectiveness
Participate in quality improvement initiatives and conduct staff training and education programs to promote excellence in patient care
Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each resident
