Coordinate patient referrals transition to home care services and assist with the collection of pertinent medical data related to the need for Durable Medical Equipment and/or supplies
Salary: $17 depending on experience

Job Qualifications
- Effective Interpersonal, time management, and organizational skills
- High School graduate or equivalent
- The ability to communicate clearly in speaking and writing
- Basic computer skills
Duties and Responsibilities
1.Present a positive professional image and promote the company in all work situations
2.Responsible for developing a good working relationship with patients, families, physicians, nurses, and other referral sources.
3.Correlates referral information on new patients to streamline communication of data related to the plan of care
4.Obtains authorization for services as needed for payors. Gather pertinent clinical information and disseminate itto clinical staff, andenter all information necessary in the computer to produce a complete patient record.
5.Obtains required clinical reports and obtains authorization extensions as necessary.
6.Participates in the coordination of deliveries to the patients homes to ensure they have cared for efficiently and conveniently.
7.Complete all paperwork as necessary for reimbursement process.
8.Participate in the Companys Performance Management program as requested
9.Pursue continuing education programs appropriate to job responsibilities
10.Perform other duties as assigned by Management.
11.Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.
12.Perform any additional duties as assigned to meet the needs of the department and organization.

Pay: $17.00 - $20.00 per hour

- This job consists of completing intake orders for our patients. The schedule is Saturday & Sunday only from 8:30am to 5:30pm. Can you adhere to this schedule without any acceptions?
