Qualifications/Experience:
- Must have a current unencumbered NYS license as a Registered Professional Occupational Therapist, Bachelor’s degree or higher.
- Graduate of an accredited school of occupational therapy or international equivalent
- Geriatric experience preferred
- A current unrestricted driver’s license and dependable transportation
- Ability to professionally communicate with internal and external resources
- Ability to function independently as well as part of a team
- Ability to function effectively in stressful and changing situations
- Ability to positively give and receive oral and written direction
Essential Job Functions:
- Evaluate residents for Occupational Therapy services upon physician order
- Develop and implement individualized treatment plans
- Provide therapeutic interventions and adjust care based on resident progress
- Document progress, treatment plans, and outcomes in accordance with regulations
- Recommend discharge from therapy services when appropriate
- Collaborate with nursing and interdisciplinary teams on mobility, ADLs, and care planning
- Participate in discharge planning and care plan meetings
- Educate residents, families, and staff on therapy-related needs
- Supervise Occupational Therapy Assistants and support staff
- Assist with equipment recommendations and procurement
- Ensure compliance with Medicare, Medicaid, and regulatory standards
- Maintain productivity and quality expectations
What We’re Looking For
- Current, unrestricted New York State Occupational Therapist license
- Bachelor’s degree or higher in Occupational Therapy
- Graduate of an accredited OT program (or equivalent)
- Geriatric experience preferred
- Valid driver’s license with reliable transportation
- Strong communication and teamwork skills
- Ability to work independently and adapt in a fast-paced environment

- Follow infection control and sanitation protocols
- Report concerns related to abuse, neglect, or safety immediately
- Maintain strong communication with the Rehabilitation Manager and team
