Job Summary
We are seeking a compassionate and dedicated Hospice Certified Nursing Assistant (CNA) to join our team in the South West area of Phoenix, near Tolleson. This full-time role provides essential patient care services within a hospice setting, supporting individuals at the end of life with dignity and respect. The ideal candidate will have experience working with diverse patient populations, including those with complex medical needs, dementia, or developmental disabilities. A strong understanding of medical documentation, vital signs, and infection control is essential to deliver high-quality care in this sensitive environment.
Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor and record vital signs accurately using EMR/EHR systems and document patient care activities in compliance with HIPAA regulations
- Provide medication administration under supervision, ensuring proper aseptic techniques and safety protocols are followed
- Support patients experiencing cognitive impairments such as Alzheimer’s or dementia through specialized memory care techniques
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient’s needs
- Educate family members and caregivers on patient care procedures and safety measures
- Maintain a clean and safe environment, adhering to infection control standards for infectious disease care and aseptic techniques
- Respond promptly to emergency situations, including acute or urgent care needs, utilizing BLS skills as required
- Assist in managing complex cases involving behavioral health, geriatrics, or pediatric patients when applicable
Requirements
- Valid CNA certification with current licensure in Phoenix, AZ
- Proven experience in hospice or palliative care settings preferred; experience in critical care, ICU, emergency medicine, or long-term care is highly desirable
- Familiarity with EMR/EHR systems such as Cerner, eClinicalWorks, or similar platforms for medical documentation and patient records management
- Knowledge of medical terminology, anatomy, physiology, and medication administration protocols
- Ability to work effectively with diverse populations including children, seniors, individuals with disabilities or developmental challenges
- Strong communication skills and compassionate bedside manner
- Ability to perform physical tasks such as lifting (including Hoyer lifts), standing for extended periods, and assisting with mobility support
- Working knowledge of HIPAA regulations and confidentiality standards
- Flexibility to work full-time hours in a fast-paced healthcare environment that values teamwork and patient-centered care
