Lead CNA Job Description
Job Overview: The Lead CNA plays a critical leadership role in delivering high-quality, client-centered home care services. This position combines hands-on caregiving, staff training, and quality assurance oversight to ensure services meet regulatory standards and align with Thrive’s holistic care model.
Responsibilities:
Direct Client Care & Coverage

Provide hands-on care to clients.

Cover open shifts as needed to ensure continuity of care.

Travel throughout service areas to support client needs.
-
Ensure client safety and maintain a safe home environment.

Report any changes in the client’s condition promptly.
Quality Assurance & Compliance
-
Develop, implement, and oversee quality control processes
-
Conduct caregiver performance evaluations and field assessments
-
Perform care plan audits to ensure compliance with regulations
-
Ensure adherence to company policies and home care standards
-
Assist with incident reporting and corrective actions
Training & Staff Development

Train and onboard Personal Care Coaches (PCCs)
-
Provide ongoing coaching and mentorship to caregivers

Educate staff on:

Best practices

Care standards

Regulatory requirements

Support continuous professional development
Care Plan Oversight

Review and evaluate individualized care plans

Collaborate with nurses and administrative staff to update care plans

Ensure services align with clients’ unique needs and preferences
Client Satisfaction & Relationship Management

Monitor client satisfaction and service quality

Address concerns and resolve issues proactively

Build strong relationships with clients and families

Foster a positive care environment
Documentation & Reporting

Maintain accurate and timely documentation of:

Client care

Caregiver performance

Quality assurance activities

Ensure all records are compliant and audit-ready
Administrative & Team Support

Assist with scheduling assessments and onboarding processes

Support administrative tasks and team initiatives

Participate in interdisciplinary team meetings

Collaborate across departments to improve service delivery
Continuous Improvement

Identify gaps in care delivery and recommend solutions

Implement initiatives to enhance quality and efficiency

Support innovation in home care practices
Skills/Qualifications:
-
Bachelor's degree in healthcare administration, nursing, or a related field OR a mix of education and experience.

Certified Nursing Assistant certification (required).

CPR and First Aid certification (required).

Must have a valid Driver's License.
-
Previous experience in a healthcare or home care setting.

Knowledge of home care regulations and industry best practices.

Excellent communication and interpersonal skills.

Attention to detail and a commitment to continuous improvement.

Familiarity with relevant technology and software for documentation and reporting.

This position does require travel throughout our service regions.
Technical Requirements:

Must own a smartphone with data.

Must own a personal computer to use for work purposes.

Must own a personal vehicle with insurance that meets minimum state guidelines.

Must be willing to travel to all our service regions.

Must be willing to report to the office as needed.
