Our Home Health Physical Therapist Assistant is responsible for assisting the physical therapist in assessing, planning, implementing and evaluating physical therapy treatments for patients. Additionally, the Physical Therapist Assistant is responsible for maintaining treatment records and patient information and communicating with other team members to ensure a comprehensive and integrated patient care plan. We are looking for a motivated and dependable Physical Therapist Assistant who will be equally comfortable with both patients and the administrative aspects of the job.
Responsibilities:
Conduct daily physical therapy evaluations and plan of care for patients.
Provide physical therapy treatments based on physician's orders.
Communicate with physicians on behalf of patients.
Perform administrative duties such as scheduling appointments; maintain medical records; communicating with other health care professionals and the public.
Maintain current knowledge and skills by attending continuing education courses and in-services.
Use appropriate techniques to assess patient's cognitive, emotional, psychomotor and functional status and promote participation in physical therapy treatment plan.
Perform therapeutic procedures under the direction of a licensed therapist or other qualified health care professional.
