Job Overview:
Summary: Under the supervision of the Senior Center Directors, the Case Worker conducts Social Service assistance in the two NYC Department for the Aging (DFTA) funded senior centers that are sponsored by CCSI - Anderson Older Adult Center & West Brighton Older Adult Center.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
1. Provide Case Assistance and Information and Referral services to senior citizens age 60+.
2. Assist seniors to apply for new and/or manage existing benefits, resources, medical appointments, etc. with the goals of helping them to maintain their health, a safe home environment and independence. Includes the completion of applications for benefits with and/or on behalf of clients.
3. Provide timely follow-up to ensure that goals have been achieved.
4. Document all assistance accordingly.
5. Maintain an accurate and comprehensive understanding of benefits, resources, and the complex social service system to provide clients with correct information and help clients navigate these systems accordingly.
6. Entry of client data, case notes, and other program data into the DFTA database(s) and other reporting platforms.
7. Ensure that all goals are maintained, and outcomes achieved as set forth by DFTA and funders.
8. Ensure that all services are provided and documented in accordance with NYC Department for the Aging standards and any standards set forth by funders and the agency.
9. Attend and participate in meetings, community events, trainings, and other program and/or agency-related activities.
10. Other duties as assigned.
Position Type and Expected Hours of Work:
This is a full-time position. Days and hours of work are generally 7:30 AM to 3:30 PM. Alternative hours may be required to attend meetings/trainings, meet program deadlines, or meet client needs.
Working conditions and physical demands required:

· Ability to work with the public and at times, with clients in stressful situations in need of assistance.
· Remain in a stationary position at a workstation and use a computer and telephone upwards of 95% of the time.
· Ability to report to two different work sites based on a schedule established and coordinated by two program Directors.
Qualifications
Education and/or experience required:
· Bachelor’s degree with at least two years of experience in the social services field, including service to vulnerable populations such as senior citizens.
Skills, Licenses, and/or competencies required:
· Excellent interpersonal skills & a pleasant demeanor
· Excellent written and verbal communication skills
· Knowledge of social service delivery systems, benefits, and resources
· Computer proficiency
· Good organization, attention to detail, and time-management skills
· Individual must possess the ability to work well independently as well as part of a team
Bilingual English/Spanish required
