Job Summary
Saba Home Health Care is a company that is professional and dedicated to a mission to deliver high-quality home health care to all in need. We are currently looking for excellent Registered Nurses (RNs) to join our growing team for part-time or per-diem supervisor positions. This position would be responsible for providing nursing supervisor duties to our clients and employees while demonstrating our values of integrity, caring and compassion.
$50 an hr
Responsibilities and Duties
- Provide assistance with the planning, coordination, leading and evaluating of home health services
- Provide assistance to the Director of Nursing with the coordination and management of home health services
- Act on behalf of the Director of Nursing as a liaison with the medical community, clients and agency personnel
- Ensure adherence to policies and procedures
- Open new cases/conduct admissions
- Identify and evaluate client needs and appropriateness of home health care
- Initiate and review clinical content, diagnoses, medical history, medications and other statuses from client records
- Provide internal case management
- Conduct client visits as needed
- Perform in-home supervisory visits of clinical staff as needed
- Mentor, develop and supervise field staff (RNs, LPNs, CHHAs) to enhance the delivery of services and professional development
- Provide daily supervision of nurses and home health aides to ensure quality of service is in line with the required plan of care
- Execute staff performance reviews during probationary periods and annually with the Director of Nursing
- Provide assistance with the orientation of new employees
- Maintain confidentiality of medical records, personnel records, etc., within the scope of their position
- Participate in any required functions in the absence of, or request by, the Director of Nursing
- Perform other duties as assigned in a professional matter
Qualifications and Skills Must be a Registered Nurse (RN) licensed in New Jersey and have worked as an RN for at least two years
- A minimum of 1 year home health care or equivalent experience is required
- Trach/Vent/G tube experience is a plus
- Prior supervisory experience preferred (clinical oversight, interviewing, competency evaluations, etc.)
- Ability to demonstrate self-confidence and positive attitude toward self and others
- Customer-service oriented
- Strong organizational, interpersonal, assessment and PC skills
- The ability to communicate well, both verbally and in writing
