We are seeking a compassionate and dedicated Nurse (RN or CNM) to join our Outreach team. This position provides community‑based nursing services to support pregnant, postpartum, and parenting individuals and families, with a strong focus on reproductive, maternal, and infant health.
You will work directly in homes, schools, community settings, and our center, helping reduce health disparities through education, relationship‑building, and collaboration with community partners.
This is an ideal role for a nurse who values prevention, education, equity, and family‑centered care.
Key Responsibilities
- Support and educate pregnant and postpartum individuals through one‑on‑one visits and group programming
- Provide home visiting services, including screening, assessment, education, and family coaching
- Lead classes and groups on reproductive health, safe sex, consent, and healthy relationships
- Promote early childhood development, safe parenting practices, and family resilience
- Assist families with non‑medical needs as appropriate and within scope
- Collaborate with medical providers, social services, and community organizations
- Maintain accurate documentation and data reporting in compliance with internal and State requirements
- Uphold client confidentiality and professional boundaries at all times
Qualifications
- Active Registered Nurse (RN) or Certified Nurse Midwife (CNM) license
- Valid driver’s license and ability to travel locally
- Strong communication, interpersonal, and organizational skills
- Ability to work independently in home‑ and community‑based settings
- Experience with outreach, family services, public health, or community nursing preferred
- Empathetic, nonjudgmental, and culturally responsive approach
