Under the supervision of the Manager or Senior Manager, the Community Direct Support Professional (DSP) provides habilitation services to individuals in community-based settings to support skill development, independence, and meaningful community integration. Services focus on assisting individuals with activities of daily living and building skills related to communication, socialization, transportation, problem-solving, money management, life safety, and other areas that promote independence and valued outcomes.
What you'll actually be doing:
- Support individuals in identifying and achieving personal goals, developing life skills, increasing independence, and participating in meaningful community activities.
- Advocate for individuals’ choices, rights, preferences, and valued social roles while helping them build and maintain meaningful relationships and community connections.
- Promote individuals’ health, safety, and well-being by providing person-centered support, positive behavior support, and assistance with self-care and independent living skills.
- Provide direct, least-restrictive services and respond appropriately to crises, emergencies, challenging behaviors, and identified risks in accordance with agency policies and regulatory requirements.
- Collaborate with individuals, families/support networks, and the interdisciplinary team to develop and implement Life Plans, Staff Action Plans, safeguards, and other supports that promote desired outcomes.
- Maintain accurate and timely documentation, report suspected abuse or neglect as required, participate in team meetings and required trainings, and ensure compliance with agency and regulatory standards.

What we're looking for:
- High school diploma or equivalent; related experience preferred.
- Must be at least 18 years of age; required training is provided upon hire as needed.
- Demonstrates effective written and verbal English communication, basic writing and computer skills, and strong organization and efficiency.
- Works effectively in a team environment with flexibility, strong communication, and the ability to anticipate needs and keep others informed.
- Maintains a valid driver’s license and meets agency driver requirements; must be reachable by phone and understand that loss of driving privileges may affect work location, schedule, hours, or employment.
