Clinical Magnet, a division of ICON Medical Network, is looking for a compassionate, dedicated Nurse Practitioner to join our partner's full-time team near Eagle Pass, TX. This opportunity is a unique position that involves working with the geriatric population within a SNF/LTAC setting. Nurse Practitioner Responsibilities
- Demonstrates both autonomy and proficiency in performance of comprehensive health assessments.
- Collaborates with members' primary care physicians to create care plans focused on prevention, early detection, and timely intervention.
- Observes the scope of practice in the state in which he or she is licensed and practicing.
- Communicates with physicians and family members during transitions of care.
- Effectively communicates with all members of the care team and facilitates medically appropriate and effective plans of care to avoid unnecessary duplication of testing and fragmentation of care.
- Participates in case rounds to review skilled and hospitalized members.
Qualifications
- Currently licensed as an active, unencumbered Acute, Adult/Gerontological, or Family Nurse Practitioner in the state of practice.
- Current unrestricted RN license for the state in which the provider is practicing.
- Basic Cardiac Life Support (BCLS) certification.
- Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice).
- Experience in skilled nursing or long-term care settings required (new NP graduates may be considered with a strong RN background in long-term care or skilled nursing).
- Case management experience preferred.
- Knowledge of current standard medical practices and experience with Medicare/Medicaid.
- Knowledge of I-SNP MOC and I-SNP population.
- Geriatric experience preferred.

- Salary starting at $118,000+ annually (higher compensation available based on experience)
