We are seeking a
Case Manager who is adaptable, solution-focused, and comfortable working in a fast-paced, high-needs environment.
What You'll Do:-
Manage a caseload of approximately 20-25 participants, providing short term, goal-oriented case management

Complete intake assessments and develop individualized service plans focused on housing navigation, healthcare access, mental health support, income stability, and life skills

Meet with participants regularly to track progress, address barriers, and support rapid stabilization
-
Coordinate and refer participants to community resources (DPSS, SSI, healthcare providers, food assistance, housing programs, etc.)

Respond to urgent and crisis driven client needs while maintaining firm, professional boundaries

Support participants in developing routines, problem solving skills, and readiness for permanent housing
-
Collaborate closely with on-site staff to ensure program compliance, safety, and continuity of care

Participate in team meetings and trainings as required
What We're Looking For:-
Strong ability to function effectively in an on-site, high traffic environment with frequent interruptions

At least 2 years of experience working with vulnerable populations, preferably in interim housing, homelessness services, shelters, or social services

Clear communication, strong documentation skills, and sound judgment
-
Emotional resilience and the ability to manage crisis oriented interactions
-
Education in social work, human services, or a related field is preferred but not required
Pay Details: $23.00 to $26.00 per hour
