Medical Assistant – Chronic Care Coordinator
Overview
We are a primary care clinic seeking a compassionate Medical Assistant to join our team as a Chronic Care Coordinator. In this role, you will support patients with chronic health conditions by providing regular phone call check-ins, coordinating their care, and keeping them connected with our providers between visits.
Key Duties
- Call and follow up with patients who have chronic conditions
- Review medications, upcoming appointments, and health concerns
- Document conversations in the electronic health record
- Update care plans and communicate important information to providers
- Provide encouragement and education for patients managing chronic conditions such as diabetes, hypertension, and heart disease
Qualifications
- Medical Assistant certification or relevant clinical experience
- Bilingual (English/Spanish) – required
- Strong computer and typing skills – required
- Excellent communication skills and a caring, patient-first attitude
Job Type: Full-time
Pay: $13.00 - $16.00 per hour
