Golden Empire Nursing and Rehab Center is actively seeking an RN Care Manager for PM shift. This position reports to the DON.

The primary role of the Registered Nurse (RN) is to monitor patients, implement treatments, develop patient care plans for long term needs, administer medications, follow up with families and provide the highest quality of care to all patients while staying in compliance with current federal, state, guidelines, and company’s regulations.
- Responsible for managing the nursing care of assigned residents and ensure maximum independence and self-direction
- RNCM will coordinate nursing functions and tasks for the residents with Staff nurses, Nursing Coordinator, physicians, other health care providers and other members of the IDT team
- Reviews all resident issues identified on alert charting to assure proper assessment and follow-up interventions are completed, care planned and resolved
- Participates in all assigned meetings and in-services
- Participates in facility survey as required
- Other responsibilities as delegated by DON
- Responsible for handling staff that calls in and replacing staff when staff coordinator not in facility
