Case Manager
Responsibilities Include:
- Conduct assessments to determine client needs and develop individualized service plans.
- Coordinate with community resources to provide clients with access to necessary services.
- Maintain accurate and up-to-date client records and documentation.
- Advocate for clients and assist them in navigating social services systems.
- Facilitate group sessions and workshops to support client development and well-being.
- Collaborate with shelter staff and external agencies to ensure comprehensive care.
- Bachelor's degree in social work, psychology, or a related field preferred.
- Experience working in a homeless shelter or similar environment.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of community resources and social services systems.
