RN Case Manager (Home Health) – Lexington, KY
Location: Lexington, KY (On-site/field-based)
Schedule: Monday–Friday, 8:00 AM–5:00 PM; on-call required 1 weekend per month + 1 day each week
Pay: $56.14/hour + mileage reimbursement

Responsibilities
- Perform comprehensive assessments for high-risk members in the home health setting
- Collaborate with primary care providers and interdisciplinary teams to implement individualized care plans
- Coordinate transitions of care with physicians, social workers, discharge planners, and other partners
- Identify health promotion and illness prevention opportunities; intervene to prevent adverse outcomes
- Track and document clinical, functional, and fiscal outcomes; maintain accurate records
- Put non-medical supports in place to improve treatment plan compliance (transportation, housing, resources)
- Manage a territory across multiple counties with mileage reimbursement
Qualifications
- Active, unrestricted Registered Nurse (RN) license (Kentucky or compact)
- Case Management certification (commonly CCM, ACM/ACM-RN, or RN-BC/CMGT-BC)
- Recent Home Health RN case management experience
- Strong OASIS documentation experience (SOC/recertification/ROC knowledge preferred)
- Hands-on HCHB (Homecare Homebase) experience
- Strong clinical judgment, communication, organization, and documentation skills
- Able to participate in on-call rotation as listed
