Letort Spring Nursing and Rehab is seeking a dedicated and compassionate Physical Therapist to join our rehabilitation team. We offer separate Full-Time and Weekday PRN opportunities, providing flexibility while making a meaningful impact on resident care.
The Physical Therapist evaluates residents, develops individualized treatment plans, and delivers therapy services that promote functional independence and improved mobility. This role requires strong clinical judgment, a patient-centered approach, and collaboration with the interdisciplinary care team.
- Provide physical therapy services in accordance with established standards of practice
- Conduct evaluations and develop individualized treatment plans with measurable goals
- Implement therapeutic interventions to improve strength, balance, and mobility
- Collaborate with nursing and therapy teams to ensure coordinated care
- Maintain accurate and timely clinical documentation
- Active Physical Therapist (PT) license in the applicable state
- Current BLS certification
- Skilled nursing or geriatric experience preferred
