<Responsibilities>
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs and observing for any changes in condition
- Maintain a clean, safe environment through regular cleaning and sanitation tasks
- Document patient progress, update care plans, and communicate effectively with healthcare teams to ensure continuity of care<Requirements>
- Prior experience in home health, assisted living, nursing home, or group home settings is highly preferred
