JOB SUMMARY
BrightStar Care of Arlington is hiring a skilled Registered Nurse (RN) to serve as a Clinical Care Coordinator, a leadership-focused role comparable to an RN Case Manager or Clinical Supervisor in home health.
This position combines clinical oversight, case management, staff supervision, and care coordination in a high-quality home health environment.

KEY RESPONSIBILITIES
RN Case Management & Care Coordination
-
Perform initial, ongoing, and change-of-condition assessments
-
Develop and update individualized plans of care
-
Coordinate interdisciplinary care
-
Lead care conferences with clients and families
-
Conduct home visits and reassessments
Clinical Supervisor Responsibilities
-
Provide clinical supervision to caregivers and nurses
-
Mentor, orient, and coach field staff
-
Ensure adherence to clinical standards and best practices
-
Support staff education and competency validation
Compliance & Quality Assurance
-
Ensure compliance with Texas Health and Human Services (HHSC), CMS, and state and federal regulations
-
Maintain accurate clinical documentation
-
Uphold HIPAA and infection control standards
-
Stay current on home health best practices
Agency & Community Support
-
Support referral relationships
-
Collaborate with leadership on clinical operations
-
Provide clinical coverage as needed
REQUIRED QUALIFICATIONS
-
Current, unrestricted Registered Nurse (RN) license in Texas
-
Home health, RN case management, or clinical supervision experience
-
Strong knowledge of Texas and federal home health regulations
-
Proven leadership and coaching skills
-
Excellent assessment and documentation abilities
-
Reliable transportation and valid driver’s license

Preparedness for Joint Commission accreditation processes and standards
PREFERRED QUALIFICATIONS

RN Case Manager or Clinical Supervisor experience

Medicare home health knowledge
-
QA or policy development experience
-
Home health, hospice, or senior care background
