Responsibilities:
- Supervise and coordinate the activities of caregivers and other staff members on weekends, ensuring that all residents receive quality care and services
- Develop and implement individualized care plans in collaboration with the healthcare team
- Conduct regular rounds to monitor resident care and safety, addressing any concerns or issues promptly
- Participate in quality assurance and quality improvement initiatives to ensure the highest standards of care
- Collaborate with other departments to ensure effective communication and seamless care transitions
- Provide guidance and mentorship to caregivers, promoting a positive and respectful work environment
- Maintain accurate and up-to-date records of resident care and services
- Identify and report any potential issues or concerns to management in a timely manner
Requirements:
- High school diploma required; degree in a related field preferred
- At least 0 years of experience in a healthcare setting, with a focus on long-term care
- Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment
