Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. The ideal candidate will possess a strong foundation in nursing practices, with experience in various clinical environments including long-term care, assisted living, hospitals, and outpatient clinics. As an LPN, you will play a vital role in delivering safe, efficient, and personalized care while supporting interdisciplinary teams and ensuring compliance with healthcare regulations. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately following physician orders and established protocols.
- Assist with medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes.
- Monitor patients' conditions closely, recognizing signs of acute pain, infection, or deterioration requiring immediate intervention.
- Maintain detailed and accurate medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA regulations.
- Provide compassionate patient education on medication management, disease processes such as dementia or Alzheimer's care, and post-discharge instructions.
- Collaborate effectively with physicians, case managers, and other healthcare professionals to coordinate patient care.
- Support patients with developmental disabilities or behavioral health needs through specialized communication and tailored interventions.
Skills
- Proficiency in using EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient management.
- Demonstrated experience in critical care environments including ICU or Level I trauma centers is highly desirable.
- Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10).
- Skilled in performing IV infusion therapy, catheter care, phlebotomy, and basic life support (BLS).
- Experience working with geriatric populations in settings such as skilled nursing facilities or memory care units.
- Ability to handle emergency situations effectively through first aid and airway management skills.
- Familiarity with infection control protocols and aseptic techniques to ensure patient safety.
- Excellent caregiver communication skills to educate patients and families about treatment plans and health coaching strategies.
