JOB DESCRIPTION
POSITION TITLE: Community Habilitation Coordinator
CLASSIFICATION: Non-Exempt
POSITION DESCRIPTION: BRIDGES operates a variety of programs guided by the philosophy of Independent Living to provide services and supports for individuals with disabilities so that they may experience autonomy in all areas of living. The coordinator will be responsible for ensuring all staff documentation meets program, agency, and OPWDD compliance standards through regular review, feedback, and coordination with supervisory staff and human resources.
- Highschool diploma required and a minimum 2 years' experience in Human Services or Customer Service field
- Familiar with Community Habilitation program requirements and regulations a must
- Provides person-centered, need-based program education and guidance to consumers and authorized representatives specific to individual choices, goals, and desired outcomes.
- Responsible for learning program policies to ensure ability to respond to customer service inquiries.
- Conduct New Hire Orientations for Caregivers in the Community Habilitation Program
- Develop staff action plans on behalf of the Consumer that aligns with Care Manager goals
- Maintain up to date knowledge of OPWDD Community Habilatation regulations to ensure the program is compliant
- Verify time worked using the HHA app and upload all approved notes to HHA ( Monthly, weekly)
- Review daily notes submitted by Community Hab worker & all monthly notes to ensure they meet program standards and compliance requirements
- Ensure that goals listed in notes align with each individual’s Staff Action Plan (SAP).
- Read and confirm that all note descriptions meet OPWDD compliance standards.
- Participate in ongoing training with management to understand what constitutes an appropriate and compliant daily note
- Process and input all administrative background requirements ( fingerprinting, criminal, etc)
- Learn the required documentation that is needed to enroll a client
- Contact staff to review and provide feedback on notes that require improvement.
- Attend professional development (webinars, workshops, conferences, etc.) to advance knowledge on related programs/activities of the organization as requested by the employee or the Director.
- Capable of communicating with and in a manner that is culturally sensitive to individuals with disabilities
- Working knowledge of Microsoft Office Software, including Excel, Word and Power Point.
- Ability to become proficient on other software products as utilized by the position
- Excellent interpersonal, organizational, written and oral communication skills; able to interact with all levels of associates in a respectful, confidential manner
- Effective and collaborative team player
- Maintain strong work ethic and strict adherence to code of ethics, standards of conduct and confidentiality requirements.
Pay: $25.00 - $27.00 per hour
