Choice Recovery/Choice Health Integrated is seeking a full-time Licensed Mental Health Counselor to provide individual and group counseling services for adults with mental health, substance use, and Serious Mental Illness (SMI). Position Summary
The Licensed Mental Health Counselor serves as a Primary Therapist, providing individual, group, and family counseling services to individuals with mental health, co-occurring substance use disorders, and Serious Mental Illness (SMI). This position requires strong clinical assessment and diagnostic skills, experience collaborating within a multidisciplinary treatment team, and the ability to provide trauma-informed, recovery-oriented care.
The Primary Therapist is responsible for guiding clients throughout the continuum of care—from admission through discharge—while coordinating with psychiatric providers, nursing staff, case managers, and other members of the integrated care team to ensure high-quality clinical outcomes.
Essential Responsibilities
- Conduct comprehensive behavioral health assessments and establish DSM-5 diagnoses.
- Develop individualized, person-centered treatment plans that address mental health, substance use, and SMI-related needs.
- Provide evidence-based individual, family, and group counseling services.
- Facilitate psychoeducational and therapeutic groups.
- Monitor client progress and modify treatment interventions as clinically indicated.
- Collaborate closely with psychiatric providers, medical staff, case managers, and other interdisciplinary team members to coordinate integrated care.
- Complete all clinical documentation accurately and within required timeframes, ensuring compliance with AHCCCS, state licensing, accreditation, and organizational standards.
- Participate in treatment team meetings, case consultations, and discharge planning.
- Recognize and respond appropriately to behavioral health crises while following agency protocols.
- Maintain confidentiality and professional ethics in accordance with HIPAA and applicable regulations.
QualificationsRequired
- Master's Degree in Counseling, Social Work, Psychology, or a related behavioral health field.
- Associate or Independent Arizona behavioral health licensure (LAC, LPC, LMSW, LCSW, LAMFT, LMFT, or equivalent).
- Arizona DPS Level I Fingerprint Clearance Card.
- Current CPR/First Aid Certification.
- Minimum of one (1) year of experience providing behavioral health services.
Preferred
- Experience working with individuals diagnosed with Serious Mental Illness (SMI).
- Experience treating co-occurring mental health and substance use disorders.
- Experience conducting behavioral health assessments, treatment planning, and crisis intervention.
- Training in trauma-informed care and evidence-based therapeutic interventions.
- Knowledge of recovery-oriented practices, integrated care models, and community behavioral health services.
- Experience documenting within an electronic health record (EHR).
- Strong communication, organizational, and interdisciplinary collaboration skills.
