RN Needed for a DDD Nursing-Supported Group Home

We are hiring an RN for a DDD nursing-supported group home. This role is perfect for a nurse who enjoys getting to know the members, supporting daily routines, and working with a team that values dignity, safety, and quality care.
This is not a rushed hospital floor. This is relationship-based nursing in a home environment where your attention to detail, calm presence, and clinical judgment matter every day.
What You’ll Be Doing
As the RN, you will provide nursing support and help guide care in the home, including:
- Providing nursing support and intervention in the group home
- Following each member’s Acuity Plan, medical orders, PCSP, Behavior Plan, and individualized healthcare routines
- Monitoring and reporting changes in condition
- Completing vital signs and other required medical monitoring
- Administering or supervising medications according to current orders
- Reviewing medication documentation and reporting discrepancies
- Supporting seizure monitoring, allergy response, diet instructions, fluid needs, and other medical routines
- Assisting with personal care, repositioning, feeding, hygiene, and direct care as needed
- Supporting caregivers with care instructions, documentation, and follow-through
- Communicating with the Director of Nursing and team members
- Completing nursing notes, medication documentation, incident reports, and other required documentation before the end of shift

Requirements
- Current Arizona RN license in good standing
- Must immediately report any change in license status
- Must be able to obtain and maintain a Class 1 Fingerprint Clearance Card
- Must be able to complete required trainings, including Article 9, Prevention and Support, CPR/First Aid, and Abuse and Neglect
- Must provide baseline TB documentation before providing services and annual documentation of freedom from symptoms of infectious tuberculosis thereafter
- Must be able to read, write, and communicate clearly in English
- DDD, group home, long-term care, behavioral health, or medically complex care experience is preferred

Pay: $33.00 to $38.00 per hour
