Community Liaison – Assisted Living Facilities & Home Health
Unified Care Services (UCS) — Doral, FL
Position Summary
The Community Liaison is responsible for building and maintaining strong relationships with Assisted Living Facilities (ALFs), physician offices, discharge planners, and other referral sources to drive census growth for UCS's services. This role serves as the face of UCS in the community — educating partners on our services, identifying new referral opportunities, and ensuring a seamless intake experience for every patient referred.
Key Responsibilities

Develop and maintain relationships with ALFs, physician offices, hospitals, and other referral sources across Miami-Dade and Broward County

Conduct regular in-person visits to assigned facilities/accounts to promote UCS services and maintain top-of-mind presence

Identify and pursue new business development opportunities to grow referral volume

Educate community partners on UCS's Home Health and DME/pharmacy service lines, eligibility criteria, and referral process

Coordinate closely with Intake, Clinical, and Operations teams to ensure timely and accurate processing of referrals

Track outreach activity, referral sources, and conversion outcomes; report on territory performance

Represent UCS at community events, health fairs, and networking functions

Resolve any service concerns raised by referral partners in a timely, professional manner

Maintain up-to-date knowledge of industry regulations, competitor activity, and market trends in the ALF and home health space
Qualifications

2+ years of experience in community liaison, marketing, or business development role within home health, hospice, ALF, or healthcare services

Established relationships with ALFs and/or referral sources in Miami-Dade and/or Broward County strongly preferred

Excellent interpersonal, communication, and presentation skills

Self-motivated with strong time management and territory planning skills

Valid Florida driver's license and reliable transportation (frequent local travel required)

Bilingual (English/Spanish) preferred

Knowledge of pediatric home health services a plus
