Position Summary
At Home Harmony (AHH) is seeking a highly motivated Community Relations & Business Development Representative to support the growth of our in-home primary care, dementia care, transitional care management, and GUIDE Model services throughout the Asheville market.
This individual will serve as the face of At Home Harmony within the community, developing and maintaining relationships with referral sources, healthcare organizations, senior living communities, hospitals, skilled nursing facilities, home health agencies, and community partners. The ideal candidate is a self-starter with strong networking skills, healthcare sales experience, and a passion for improving access to care for older adults and individuals living with chronic conditions.
Essential ResponsibilitiesBusiness Development
- Develop and execute territory growth plans for the Asheville and surrounding service areas.
- Identify and cultivate referral relationships with:
- Assisted Living Communities
- Memory Care Communities
- Independent Living Communities
- Skilled Nursing Facilities
- Hospitals and Health Systems
- Home Health Agencies
- Hospice Providers
- Rehabilitation Facilities
- Community-Based Organizations
- Conduct routine in-person visits with referral sources.
- Generate qualified patient referrals for At Home Harmony services.
- Maintain consistent referral source communication and follow-up.
- Identify opportunities for strategic partnerships and community outreach.
Community Outreach & Relationship Management
- Represent At Home Harmony at community events, health fairs, educational seminars, and networking opportunities.
- Develop relationships with community leaders and healthcare decision-makers.
- Coordinate educational presentations regarding:
- In-home Primary Care
- Dementia Care Services
- CMS GUIDE Program
- Transitional Care Management
- Caregiver Support Resources
- Promote At Home Harmony's mission and value proposition within the community.
Referral Management
- Track referral activity and conversion rates.
- Collaborate with intake, scheduling, and clinical teams to ensure seamless onboarding of new patients.
- Assist referral sources with understanding service eligibility and admission requirements.
- Follow up on referral outcomes and provide timely communication to referral partners.
Strategic Growth Initiatives
- Assist leadership in evaluating market opportunities and competitive positioning.
- Identify underserved populations and growth opportunities.
- Participate in strategic planning and market expansion initiatives.
- Support the launch and growth of new service lines.
CRM & Reporting
- Maintain accurate records of all referral activities.
- Document meetings, outreach efforts, and referral outcomes.
- Submit weekly activity reports and monthly territory summaries.
- Monitor referral trends and identify opportunities for improvement.
Performance Expectations
Success in this role will be measured by:
- New referral source development
- Referral volume growth
- Patient admissions generated
- Referral-to-admission conversion rates
- Community engagement activities completed
- Relationship retention with key partners
- Market growth within assigned territory
QualificationsRequired
- Bachelor's degree preferred or equivalent experience.
- Minimum of two (2) years of healthcare sales, physician relations, community relations, or business development experience.
- Strong understanding of healthcare delivery systems.
- Excellent communication and presentation skills.
- Ability to work independently and manage a territory.
- Valid driver's license and reliable transportation.
Preferred
- Experience with senior care, home-based primary care, home health, hospice, or assisted living environments.
- Existing relationships within the Asheville healthcare market.
- Knowledge of Medicare and value-based care programs.
- Familiarity with dementia care and caregiver support programs.
