Order medications, communication with pharmacy staff for medication issues
Take Vital signs per protocol and as needed - input in KIPU
Assess Cows/Ciwas per protocol and as needed - input in KIPU
Overseeing self-administration of scheduled and prn medications
Medication – orders, refills, adjustments, changes, centrally store, input, witness/verify medications brought in.
Controlled Medications – log in Controlled book, centrally store, input as taper, upload of receipts and completed controlled logs into medical tab
Upload Client medical documents/discharges into medical tab
Input Client infection control for dx/medications in monitoring log
Nursing progress notes – admission, residential discharge, final discharge, change of condition, medication change, additions, discontinuations, and client teaching
Final and Residential Discharge – communicate with the provider for discontinue or continue of medications in SL or home environment, add current medications and Nursing Progress notes
Daily communication with Director of Client Care for updates/issues/endorsements
Carry out medication changes provided in assessments
Monitor controlled logbook for errors
Maintains professional standards and follows the treatment policies of the organization, The Joint Commission and Federal/State Laws and Regulations
The LVN may be responsible for urine collection and processing in conjunction with the clinical technicians, reading test results as ordered by Program Physicians and the completion of all forms involved in these procedures
The LVN is also responsible for coordinating all internal and external medical and lab appointments
Pay: $28.00 - $35.00 per hour
