Job Summary: This individual coordinates Client Services, Caregiver Services and Community Relations. The Case Manager is also involved in client service issues such as evaluations and supervisory visits with regard to their interface with the aforementioned primary functions. Responsibility includes client case management, customer satisfaction and managing other responsibilities as needed.
Qualifications:
Required:
· Must be at least eighteen years old and must be a high school graduate with some college or business education preferred.
· One or two years’ experience in an office environment, preferably in a healthcare setting.
· Strong customer service focus and excellent interpersonal communication skills.
· Pleasant telephone presence that conveys warmth, enthusiasm, and friendliness.
· Able to think quickly and use creative solutions when needed.
· Experience with scheduling employees in a shift work environment.
· Strong attention to detail and excellent organizational skills.
· Ability to communicate effectively in English – verbal and written.
· Able to follow written and verbal instructions competently.
· Able to effectively utilize computer system for tracking and managing agency activities. Visiting Angels will provide training on their specific software applications.
· Ability to maintain accurate schedules, notes and records.
· Demonstrate a sense of trustworthiness and ability to maintain strict confidentiality regarding sensitive client, employee, and other agency information.
· Present a professional image and conduct.
· Must meet all Agency requirements, including but not limited to passing a thorough criminal background check, social security number verification and pre-employment drug screen.
· Have a sincere interest and desire to work with the elderly.
· Have a reliable vehicle and a valid driver’s license.
Preferred:
· Case Management experience.
Duties and Responsibilities:
· Coordinate with Director or Designee for the development and administration of programs, service procedures and policies as needed.
· Provide for safe and appropriate services in accordance with written service plans, under review of Director or Designee
· Comprehensive case management for assigned clients.
· Assists with employee recruitment, hiring and orientation activities.
· Screen individuals providing services for the agency to determine that they meet the qualification, orientation, competency, training, and education requirements in the WACs
· Manage personnel and client care practices consistent with the agency's written policies and procedures under review of Director or Designee
· Implement client care assignments based on the caregiver's abilities, skills, and competence under review of Director or Designee
· Ascertain that agency does not accept or retain clients for whom it does not have the capabilities or resources to provide services under supervision of Director or Designee
· Review Client Service Plan with Caregivers.
· Provide and maintain all written documentation for supervisory visits and calls.
· Responsible for personnel management.
· Sets up and maintains all client files.
· Document Client and caregiver changes electronically and in printed files/notebooks.
· Receives and resolves Client/client complaints, as necessary.
· Attend and participate in community
· Implementation of quality improvement including participation in monthly meetings
· After-hours on-call telephone coverage, on a rotating basis.
· Travel as needed.
· Cross train into other departments and fill-in as needed.
· Other duties as assigned.
