We are looking for Full-time LPN 1:30p - 10:30p with every other weekend off (weekend is 3 days)!!!

We are a 24 bed facility with a great staff to resident ratio. During wake hours we have 7-8 direct support professionals along with a house manager that is responsible for supervising DSPs, answering the phones, taking care of visitors, and any building issues freeing you up to focus solely on your nursing duties.
Licensed Practical Nurse Job Description
A Licensed Practical Nurse will serve to meet all health and safety concerns of individuals; providing basic patient care, working under the supervision of physicians and the DON, and supervising Direct Support Professionals.
Duties include, but are not limited to:
- The administration and documentation of prescribed medications and treatment
- Assessing and monitoring health status
- Providing timely and appropriate nursing care
- Transcribing physician’s orders according to Nursing Procedures
- Maintaining detailed nursing notes
- Participating in the monthly changeover of medications and treatments
- Working collaboratively with the interdisciplinary team to ensure quality care is provided
- Providing direct care to individuals as needed
