Under general supervision, provides professional nursing care in accordance with established nursing protocols, policies, and procedures. Focuses on disease prevention, health promotion, care coordination, and maintenance of the health of individuals, families, and populations. Performs clinical assessments, develops plans of care, provides treatment and health education, coordinates services, and collaborates with healthcare providers and community partners to improve health outcomes.

- Performs comprehensive health assessments and evaluates client healthcare needs.
- Develops, implements, and modifies plans of care based on assessment findings.
- Provides treatments, medications, immunizations, and other nursing interventions in accordance with established protocols.
- Delivers preventive health, maintenance care, and health promotion services.
- Provides health education and counseling to individuals and families.
- Screens clients and coordinates referrals to healthcare providers, specialists, and community resources.
- Participates in care coordination and case management activities.
- Maintains accurate clinical documentation, reports, and records.
