Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and observe patients’ conditions by performing patient observation, recording vital signs, and reporting changes promptly to healthcare professionals.
- Administer medications and treatments following medical documentation protocols, ensuring compliance with home healthcare regulations and HIPAA confidentiality standards.
- Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and end-of-life assistance.
- Maintain accurate medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; document patient care activities thoroughly.
- Prepare nutritious meals and assist with meal preparation while adhering to dietary restrictions and safety guidelines.
- Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment to promote overall health and well-being.
Skills
- Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring long-term or home health care.
- Strong knowledge of medical terminology, patient personal care assistance, catheter care, and vital signs measurement.
- Familiarity with home health agency operations, hospice services, and community-based caregiving practices.
- Ability to communicate effectively with clients and team members while maintaining patient confidentiality under HIPAA standards.
- Competence in using EMR/EHR systems for medical documentation and client communication.
- Knowledge of CPR, First Aid, Basic Life Support (BLS), and other emergency response procedures.
- Physical stamina for heavy lifting, patient transfer methods such as Hoyer lifts or other assistive devices, and working in diverse settings including assisted living facilities or private homes.
- Experience working within home healthcare regulations and providing compassionate end-of-life or hospice support when needed.
