Job description:
The Outreach/Case Worker will engage with the homeless community, both in the field and in the office. There are two separate components to this position. Outreach includes traveling within the local area to places where unhoused people congregate to offer supplies, support, and referrals to other community partners. They'll work as a part of an outreach team as well as independently. Case management includes working with clients to overcome barriers to housing and providing them with skills to remain successfully housed.

Essential functions:
- The Outreach/Case Worker will provide support to participants with activities geared toward fostering independence and resiliency. This includes assisting the Director in developing action plans, navigation to community resources, and reducing barriers to achieving goals
- Works closely with partnering community programs to enhance supports for the participant and will participate in ongoing data management and reporting
- Collaborates with Director to ensure data quality and completion of reports
- Aids in gathering documentation as required for intake
- Provides direct service to participants in intensive peer support and overcoming barriers to accessing permanent housing
- Collaborates with the program participant to develop individualized action plans
- Identifies participant needs, barriers, and strengths in the creation of these household action plans
- Reviews the service plan and goals with the participant and coordinate them to services
- Provides support and assistance to participants to develop skills in Activities of Daily Living (ADL)
- Monitors participant progress and maintains detailed and up-to-date case notes
- Enters data into government database
- Ensures client files are complete and organized
- Ensures compliance with Coordinated Entry Best Practices
- Attends weekly case management meetings as well as occasional trainings
- Embraces the mission and philosophy of Trauma Informed Care and Harm Reduction strategies
- Performs tasks related to inventory control
- Organizes and supervises teams for outreach
- Reviews daily outreach reports and tallies monthly figures for harm reduction, food resources, and client demographics
Job Requirements:
- Lived Experience with mental health, substance use disorder or being unhoused.
- Ability to Lift/Pull/Carry up to 50 lbs.
- Sitting for an extended period of time.
- Standing/Walking Outdoors up to 4+ hours.
- Ability to walk up and down, up to 2 flights of stairs.
- Drivers License, Reliable Vehicle and Valid Car Insurance.
- Typing Skills 25 WPM.
- Understanding and ability to use Microsoft Office Suite.
- Adaptability to rapidly changing situations.
Pay: $22.00 - $24.00 per hour
